Online self-referral system launched for Pettis County residents

Pettis County residents now have access to a new online self-referral form designed to assist with challenges such as food, transportation, home repairs, and utilities. This initiative is part of the Transformation of Rural Community Health (ToRCH) program managed by Bothwell Regional Health Center.

The ToRCH program is sponsored by the Missouri HealthNet Division, which oversees the Missouri Medicaid program. It aims to enhance the well-being of rural Missourians by linking them with essential resources and support services. Residents can complete a straightforward self-referral form on the Bothwell website to request assistance from various community agencies.

Allison Brosch, Dean of Health Sciences at State Fair Community College and chair of the ToRCH Be Well board, highlighted that the self-referral form simplifies outreach for individuals who may be unsure where to start or what services are available. “There are so many great community-based organizations in Sedalia that provide valuable and needed social services for individuals and families in Pettis County who are approved for Medicaid,” she stated. “This online form is a one-stop place to request assistance from many of these organizations.”

The secure form is provided by Unite Us, a leading software organization facilitating collaboration across sectors to improve community health and well-being. The Missouri HealthNet Division collaborated with Unite Us to establish a new care standard that identifies social care needs, connects people with services, and utilizes meaningful outcomes data for ToRCH partners.

Information about the program and access to the form can be found at brhc.org/torch. Eligible participants should expect contact from an assisting organization within one week.
